Description of Services: Hospital-based outpatient program with a focus on geriatric rehabilitation * exercise and education activities to help older adults achieve optimal mobility, function, and independence * services provided by a physiotherapist, occupational therapist, occupational therapy/physiotherapy assistant, and speech language pathologist

Progression level of rehabilitative care for patients working to restore or maximize functionality

Eligibility - Population(s) Served: Medically stable older adults * must be able to transfer with one person or independently * must be able to learn new tasks * must have the potential to improve functionality * must be motivated and willing to participate * must arrange own transportation
