Eligibility - Population(s) Served: Ontario residents 60 years and older who are medically stable * must have a valid OHIP card * must be able to follow instructions and self-determine modifications suitable to personal health limitations * must be able to engage in conversation and make decisions independently * must be be able to transfer independently or with one person assisting
Application: Medical referral required for ambulatory rehabilitation
