Description of Services: 12-week, specialized in-home support program designed for isolated older adults living with dementia and currently on a waitlist for Adult Day Programs (ADS) in Halton

Program includes:
  • 4 hours per week of personalized in-home care led by trained dementia care professionals, providing enrichment and activity for the client, and respite for the caregiver
  • customized care plans tailored to each person’s interests and abilities, co-developed with the family
  • dementia-friendly activity kits for use during and beyond the visits
  • support and coaching for family caregivers to build confidence and resilience
  • dementia-friendly social events

Fees: Free
Eligibility - Population(s) Served: People living with dementia who are at high risk of social isolation and rapid decline without appropriate services
