Description of Services: Community-based health care in which paramedics provide alternative to family doctor, walk-in clinic, or emergency department * provides preventative, at-home health care visits * focuses on reducing avoidable use of emergency and ambulance services * helps older adults and vulnerable, at-risk people live safely in their homes * services include:
  • health and medical risk assessments
  • chronic disease management
  • medication management
  • care coordination with primary care providers and other agencies
  • referral to community programs and resources
  • remote patient monitoring

Eligibility - Population(s) Served: High-risk clients who have complex medical issues such as diabetes, chronic obstructive pulmonary disease, and congestive heart failure
