Description of Services: Offers immediate support to seniors and older adults needing a rapid crisis response due to mental health, poverty, addictions, disabilities, and other vulnerabilities * team includes emergency food delivery, follow up supports and wellness checks * food delivery service includes emergency food, prepared meals and access to a mobile food market * intervention worker helps out with support and connection to other services
