Mississauga Halton Central Intake Program
Description of Services: Central intake for patients with low back pain * provides patients with an assessment by an Advanced Practice Provider within 4 weeks of when the completed referral is received by the Central Intake Program * an Advanced Practice Provider (APP) is a regulated healthcare professional such as a physiotherapist, registered nurse, nurse practitioner or chiropractor with additional training in orthopedic care


Objectives of the program include:
  • improve outcomes and satisfaction for patients with persistent or unmanageable recurrent low back pain
  • reduce opioid initiation through early non-pharmacological management
  • decrease utilization of lumbar spine imaging such as MRIs
  • reduce unnecessary emergency room visits for low back pain
  • reduce unnecessary referrals to low back pain-related specialists
  • improve access to specialist care

Fees: Services covered by OHIP
Eligibility - Population(s) Served: People with persistent low back pain and/or related symptoms or recurrent episodic unmanageable lower back pain * must be 18 years or older * residents of Mississauga Halton region
Application: Primary Care Providers must register and complete an online module to obtain referral privileges

About Us

As part of a provincial initiative, Home and Community Care Support Services Mississauga Halton in partnership with Halton Healthcare and Trillium Health Partners has launched Central Intake and Rapid Access Clinics (RAC) for patients with moderate to server hip and knee arthritis on November 1, 2018 to ensure equitable access to high quality care continuum for musculoskeletal care.
