Description of Services: Provides support for high-risk transitions to long-term care homes for people who exhibit responsive behaviours * transitional lead works with client's family and care providers to develop behavioural care plans as client is admitted into long-term care * works with BSO Long-Term Care Home Mobile Team to provide intensive support in first six weeks after admission
