Diabetes Services - Diabetes Education Program
Description of Services: Provides foot care education and management to individuals with diabetes who have financial barriers to accessing fee-for-service foot care * team includes a chiropodist and a foot care nurse (nurse with specialized foot care training)

Services include:
  • routine and complex nail care
  • corn and callus reduction
  • diabetic foot care
  • early wound management
  • footwear assessment and education
  • foot care and self-management education

Diabetes Education Program
  • client can self-initiate a referral and the family doctor or other health care provider will be contacted to complete the referral process
  • one-on-one assessment with a Diabetes Educator who can refer client to the Halton Diabetes Foot Care Program if eligibility criteria are met

Eligibility - Population(s) Served: Qualified patients 18 years of age and older with diabetes residing in Mississauga Halton Region who are enrolled in a Diabetes Education Program in Mississauga/Halton
Application: Referral form to be completed by Diabetes Education Program in Mississauga Halton
